Non-SCI Referral form

This form can be used by referring clinicians if there is no access to SCI gateway.

Referral forms will be accepted from Allied Health Professionals, Secondary Care Clinicians and Clinicians who have access to the patients' medical records and feel that the Long COVID service would be of benefit to their patient.

Please ensure the patient consents to the referral and that they meet the criteria listed.
